Secondary Transition Services

Successful transition from school to adult life requires assessment and instruction in all areas of the expanded core curriculum content areas, as well as, an understanding of the intended post-secondary outcome for the students’ futures.This process begins, at the latest, at the IEP when the student is age 14.  Collaboration with the student, family, team of educators and future partners in the targeted areas of living, working and learning can positively influence future outcomes.
